Thanks for your co-operation it’s important to us.

Please note: this should ideally be done before / during training sessions and matches by both home and away supporters.  This is so that in the event that an attendee tests positive, we can track everyone at the venue and let them know accordingly.

Please register your attendance (one person per family) by completing the form below. It is important that we capture this information inline with the FA and Government guidelines. Please ensure you complete all fields.

Please also note we will not use this information for any other purpose, other than for the safety of all involved during training and matches. This is in accordance with the FA & Government guidance.
